What is the Community Reintegration program?

The Community Reintegration Program provides transition assistance to persons with disabilities that are between the ages of 18 – 59 and reside in a nursing facility to locate and secure affordable, accessible housing in the community.  The goal of the program is to provide community based services for the person so they can lead an independent life in the community. 

The Community Reintegration Program is funded by the Illinois Department of Human Services / Division of Rehabilitation Services  ( DHS/DRS).  The program is also funded by Medicaid.

 

What assistance is provided for the person?

  • Assistance in locating and securing affordable and accessible housing
  • Assistance with security deposit and first months rent
  • Provide home furnishings
  • Provide any assistive equipment or adaptive technology
  • Provide independent living skills
  • Provide advocacy and peer support
  • Provide case management

 

What are the Qualifications and Eligibility Requirements for the program?

  • Reside in an Illinois nursing home
  • Be a U.S. Citizen
  • Be between the ages of  18  - 59
  • Has a disability which will last for 12 months or duration of life
  • Have  under $ 17,500 in non-exempt assets
  • Receives or is eligible for Medicaid
  • Approval of Physician for community based care options
